For refractory or recurrent giardiasis—defined as persistent symptoms or positive stool tests after two appropriate courses—evaluation for reinfection, nonadherence, drug resistance, or underlying immunodeficiency (e.g., IgA deficiency, HIV) is mandatory. In such cases, combination therapy (e.g., albendazole plus metronidazole) or extended-duration tinidazole (2 g daily for 3 days) may be considered, though evidence remains limited.
Surgical intervention has no role in the routine management of giardiasis. The parasite resides exclusively in the lumen of the proximal small intestine and does not invade tissue or cause structural complications amenable to surgery. Rare exceptions include severe, chronic malabsorptive syndromes leading to life-threatening nutritional deficits unresponsive to medical therapy—where enteral feeding tube placement (e.g., nasojejunal or percutaneous endoscopic gastrostomy-jejunostomy) may be required for nutritional support—but this addresses sequelae, not the infection itself. Endoscopic procedures are purely diagnostic in refractory cases and do not constitute therapeutic intervention.
