Nucleos(t)ide analogues (NAs) are first-line: entecavir (0.5 mg daily in nucleoside-naïve adults) or tenofovir disoproxil fumarate (TDF, 300 mg daily) or tenofovir alafenamide (TAF, 25 mg daily) are preferred due to high barrier to resistance and proven renal safety profiles—especially TAF, which achieves potent viral suppression with lower systemic exposure and reduced proximal tubular toxicity. Interferon-alpha (IFN-α) is rarely used in HBV-GN due to its proinflammatory effects and risk of exacerbating proteinuria or precipitating acute kidney injury; it may be considered only in select HBeAg-positive adolescents with preserved renal function and no contraindications, under strict nephrology-hepatology co-management. Immunosuppressants—including corticosteroids, cyclophosphamide, or calcineurin inhibitors—are generally contraindicated unless compelling evidence excludes active HBV replication (e.g., undetectable HBV DNA after prolonged NA therapy) and alternative diagnoses (e.g., primary MN) are ruled out. In rare refractory cases with severe crescentic GN or rapidly progressive decline, short-course pulse methylprednisolone (500–1000 mg IV × 3 days) followed by oral taper may be cautiously employed—but only after confirming virologic control and with concurrent NA escalation.
Surgical treatment has no definitive role in HBV-GN. Renal biopsy is diagnostic—not therapeutic—and performed percutaneously under ultrasound guidance with strict adherence to bleeding precautions (platelet count >100 × 10⁹/L, INR <1.5). Nephrectomy is never indicated for HBV-GN itself. In end-stage disease, kidney transplantation is feasible but requires meticulous pre-transplant virologic control: HBV DNA must be undetectable for ≥6 months on stable NA therapy, with anti-HBs titers ≥100 mIU/mL post-vaccination or hepatitis B immunoglobulin (HBIG) prophylaxis initiated perioperatively. Post-transplant, lifelong NA continuation is mandatory; TAF is favored over TDF to minimize bone and renal toxicity in transplant recipients.
