Can Transvaginal Ultrasound Detect Ectopic Pregnancy?

Transvaginal ultrasound is the gold-standard imaging modality for diagnosing ectopic pregnancy in early gestation. Because of its proximity to pelvic structures, this high-resolution technique provides superior visualization of the uterus, adnexa, and cul-de-sac compared with transabdominal ultrasound—particularly before 6 weeks’ gestation.

A definitive diagnosis of ectopic pregnancy requires identification of an extrauterine gestational sac containing a yolk sac or embryo with or without cardiac activity. However, in many cases, clinicians rely on indirect sonographic findings—such as an empty uterine cavity despite elevated serum β-hCG levels, an adnexal mass separate from the ovary, or free fluid in the pelvis—especially when a gestational sac is not yet visible.

It’s important to note that while transvaginal ultrasound is highly sensitive and specific, it cannot rule out ectopic pregnancy with absolute certainty in all scenarios. A negative scan in a symptomatic patient with rising β-hCG levels warrants close clinical follow-up, serial ultrasound examinations, and quantitative hCG monitoring. In some instances, diagnostic laparoscopy may be required for confirmation.

Early detection via transvaginal ultrasound significantly improves outcomes by enabling timely medical or surgical intervention—reducing risks of tubal rupture, hemorrhage, and maternal morbidity.
