One of the most frequent causes is dietary intake. Foods rich in fermentable carbohydrates—such as beans, lentils, cruciferous vegetables (e.g., broccoli, cabbage), onions, garlic, and certain fruits like apples and pears—contain fibers and sugars (e.g., raffinose, fructose, lactose) that resist digestion in the small intestine. When these reach the colon, resident gut bacteria metabolize them, producing hydrogen, methane, and carbon dioxide as byproducts—leading to increased flatus volume and sometimes odor, particularly when sulfur-containing compounds are generated.
Lactose intolerance is another prevalent contributor. Individuals with reduced lactase enzyme activity cannot fully digest lactose, the sugar in dairy products. Undigested lactose ferments in the large bowel, resulting in bloating, cramping, diarrhea, and notably, increased flatulence.
Swallowing excess air—medically termed aerophagia—can also elevate gas volume. This commonly occurs during rapid eating, chewing gum, smoking, or drinking carbonated beverages. The swallowed air accumulates in the stomach and is often expelled via belching; however, some may travel distally and exit as flatus.
Altered gut microbiota composition—dysbiosis—may further influence gas dynamics. Antibiotic use, chronic stress, or underlying conditions such as irritable bowel syndrome (IBS) can shift microbial populations toward species that produce more gas or enhance gas retention. In IBS, visceral hypersensitivity and abnormal colonic motility may heighten awareness of normal gas volumes and impair efficient transit, contributing to perceived excess.
Less commonly, persistent or worsening flatulence may signal an underlying pathology, including small intestinal bacterial overgrowth (SIBO), celiac disease, inflammatory bowel disease (IBD), or pancreatic exocrine insufficiency. Red flags warranting clinical evaluation include unintentional weight loss, nocturnal symptoms, rectal bleeding, anemia, or a family history of gastrointestinal malignancy.
In most cases, symptom management begins with dietary modification—such as a low-FODMAP trial under dietitian guidance—and mindful eating habits. If symptoms persist despite conservative measures, targeted testing (e.g., hydrogen breath testing for lactose intolerance or SIBO) and gastroenterological assessment are appropriate next steps.
