Key risk factors affecting outcomes include maternal age (>37 years), diminished ovarian reserve (AMH <1.1 ng/mL), recurrent implantation failure (≥3 prior failed transfers), endometriosis-associated inflammation, chronic endometritis, suboptimal endometrial thickness (<7 mm), and subclinical thyroid dysfunction. While FET itself is minimally invasive and carries low procedural morbidity, the broader fertility journey imposes substantial psychosocial burden: patients frequently report elevated anxiety, depression, marital strain, financial stress, and occupational disruption. The cyclical nature of monitoring, hormone injections, and repeated negative pregnancy tests contributes to emotional exhaustion and reduced health-related quality of life (HRQoL), particularly when multiple FET attempts are required. Importantly, FET offers distinct advantages over fresh transfer—including higher implantation and live birth rates in certain populations (e.g., PCOS, high responders), lower preterm birth and low birth weight incidence, and greater flexibility in timing—making it a first-line strategy in modern ART protocols. However, its efficacy remains highly dependent on integrated clinical, embryological, and endometrial diagnostics, underscoring the need for individualized, multidisciplinary care in reproductive medicine departments.
