疾病概述
Thalassemia is a group of inherited hematologic disorders characterized by defective synthesis of one or more globin chains of hemoglobin, leading to imbalanced alpha- and beta-globin production, ineffective erythropoiesis, chronic hemolytic anemia, and systemic iron overload. It arises from mutations—typically deletions or point mutations—in the HBA1/HBA2 (alpha-thalassemia) or HBB (beta-thalassemia) genes on chromosomes 16 and 11, respectively. Pathophysiologically, reduced globin chain synthesis causes unpaired chains to precipitate in red blood cell precursors, triggering apoptosis in the bone marrow (ineffective erythropoiesis) and premature destruction of mature RBCs in the spleen (hemolysis). Compensatory hyperplasia of erythroid precursors expands bone marrow, contributing to skeletal deformities, osteoporosis, and extramedullary hematopoiesis. Chronic anemia induces compensatory cardiac output elevation and tissue hypoxia; repeated transfusions—essential for severe forms—lead to progressive iron deposition in the heart, liver, and endocrine organs, causing cardiomyopathy, cirrhosis, diabetes, and growth failure. Epidemiologically, thalassemia is most prevalent in malaria-endemic regions: Southeast Asia, the Mediterranean basin, Middle East, Indian subcontinent, and Africa. Globally, over 280 million people carry a thalassemia trait; approximately 60,000–100,000 infants are born annually with transfusion-dependent thalassemia (TDT), with China reporting ~10,000–15,000 confirmed cases—concentrated in Guangdong, Guangxi, Hainan, and Yunnan provinces due to historical migration patterns and founder effects. Key risk factors include consanguineous marriage, family history of thalassemia or unexplained anemia, and ethnic ancestry from high-prevalence regions. Prenatal screening and carrier testing significantly reduce incidence where implemented. Quality of life is profoundly impacted: patients with TDT require lifelong monthly transfusions and daily iron chelation therapy, resulting in treatment burden, fatigue, delayed puberty, infertility, psychosocial stress, and reduced educational and occupational participation. Even non-transfusion-dependent thalassemia (NTDT) patients experience chronic complications including thrombosis, pulmonary hypertension, leg ulcers, and gallstones. Psychosocial support, genetic counseling, and multidisciplinary care—including hematology, cardiology, endocrinology, and psychology—are integral to optimizing outcomes. Advances in gene therapy and novel agents like luspatercept offer transformative potential, yet access remains limited in resource-constrained settings.
