Cryoglobulinemic glomerulonephritis (CGN) is a rare, immune-mediated kidney disease characterized by the deposition of cryoglobulins—abnormal immunoglobulins that precipitate at low temperatures—in the glomeruli, triggering inflammation, endothelial injury, and progressive renal damage. It most commonly arises as a renal manifestation of cryoglobulinemic vasculitis, frequently associated with chronic hepatitis C virus (HCV) infection (Type II and III cryoglobulinemia), though it may also occur in autoimmune conditions (e.g., SLE, Sjögren’s syndrome), lymphoproliferative disorders (e.g., Waldenström macroglobulinemia), or idiopathically. Pathogenesis centers on immune complex formation: cryoglobulins—typically IgM-IgG (Type II) or polyclonal IgG/IgM (Type III)—circulate in the bloodstream and deposit in glomerular capillaries upon temperature-dependent precipitation or complement activation (especially via the lectin or alternative pathways), leading to mesangial hypercellularity, endocapillary proliferation, and often crescent formation. Histologically, light microscopy reveals membranoproliferative or proliferative patterns; immunofluorescence shows dominant IgM, IgG, and C3 deposits; electron microscopy demonstrates subendothelial and intramembranous electron-dense deposits. Epidemiologically, CGN is exceedingly rare, with an estimated incidence of <1 per million annually in general populations. It predominantly affects adults aged 50–70 years, with a slight female predominance. HCV remains the strongest modifiable risk factor—up to 90% of Type II/III cryoglobulinemia cases are HCV-associated—while other risks include HIV co-infection, chronic HBV, autoimmune dysregulation, B-cell malignancies, and prolonged exposure to environmental toxins or certain biologics. Untreated CGN carries high morbidity: rapid progression to nephrotic syndrome, acute kidney injury, or end-stage renal disease (ESRD) occurs in ~30–40% within 5 years. Patients frequently experience systemic symptoms—including purpura, arthralgia, peripheral neuropathy, and fatigue—alongside renal manifestations such as proteinuria, hematuria, hypertension, and declining eGFR. Quality of life is significantly impaired due to chronic fatigue, recurrent flares requiring hospitalization, dietary and fluid restrictions, anxiety about dialysis dependence, and psychosocial burden from visible vasculitic skin lesions and treatment-related side effects (e.g., corticosteroid-induced insomnia, mood changes, or rituximab-infusion reactions). Early diagnosis—via serum cryoglobulin testing (requiring strict pre-analytical handling at 37°C), renal biopsy, and HCV RNA screening—is critical. Prognosis improves markedly with timely, etiology-directed therapy, yet long-term monitoring for relapse, lymphoma development, and cardiovascular complications remains essential.
