疾病概述
Viral gastroenteritis, commonly known as the 'stomach flu,' is an acute, self-limiting inflammation of the stomach and intestines caused primarily by enteric viruses. Despite its colloquial name, it is not related to influenza virus; rather, it is most frequently triggered by norovirus (responsible for ~50–70% of adult cases globally), rotavirus (predominant in children under 5), adenovirus, astrovirus, and sapovirus. Pathogenesis involves viral attachment to enterocytes in the small intestine, leading to villous blunting, impaired nutrient and fluid absorption, and activation of the enteric nervous system—resulting in secretory diarrhea, vomiting, and abdominal cramps. Viral replication induces localized epithelial damage and transient immune-mediated inflammation, but systemic invasion is rare. Epidemiologically, viral gastroenteritis affects an estimated 1.7 billion people annually worldwide, causing over 500,000 deaths—primarily among children under five in low-resource settings. In high-income countries like China, incidence remains high due to dense urban populations, shared dining practices, and seasonal peaks (norovirus surges in winter; rotavirus in late winter–early spring). Risk factors include young age (<5 years), advanced age (>65), immunocompromised status (e.g., post-transplant, HIV), institutional living (nursing homes, dormitories), poor hand hygiene, consumption of contaminated food or water, and travel to endemic areas. While most cases resolve spontaneously within days, complications—including severe dehydration, electrolyte imbalances (hypokalemia, metabolic acidosis), acute kidney injury, and hospitalization—are more common in infants, elderly, and chronically ill patients. Quality of life impact is substantial yet often underestimated: acute symptoms cause significant work/school absenteeism (averaging 2–3 days per episode), caregiver burden, anxiety around contagion, and disruption of daily routines. Recurrent norovirus infections—due to short-lived, strain-specific immunity—can lead to persistent psychosocial stress, especially among healthcare workers and parents of young children. In China, rising awareness and improved diagnostics have increased outpatient reporting, though underreporting remains widespread due to mild presentation and lack of routine stool testing. Public health efforts increasingly emphasize prevention—handwashing with soap (alcohol-based sanitizers are less effective against norovirus), surface disinfection with chlorine-based agents, food handler exclusion during illness, and rotavirus vaccination (included in China’s expanded program immunization since 2020 for infants). Early oral rehydration therapy (ORT) remains the cornerstone of management, reducing mortality by >90% when appropriately administered.
